Hello parents. To protect our athletes we are now requiring all parents to sign and return a web release form. This form gives, or restricts, permission for us to put pictures, images or movies, of your athlete on this web site.
The release form is provided in standard HTML or Adobe Acrobat PDF format. If you don't have Adobe Acrobat installed, there is a download link below, for your convenience. Here are the links to the web releases:
Here's what we need you to do:
Download and print the release form.
Choose the appropriate option regarding pictures and last names.
Sign and date the form and return to:

There is an second section below the permissions, which will ultimately be used to create player profile pages. Please take a little time and fill this section out with your student. This section is a great place to "brag" about your athlete and their accomplishments outside of the water polo arena.
